What you capture
Phrase
The exact words your child said—or as close as you can remember.
Context
What was happening when they said it. Where were you? What triggered it?
Tags
Simple labels to help you find entries later. Use what makes sense to your family.
Date
Defaults to today. Change it if you're logging something from yesterday.
Why context matters
The same phrase can mean different things in different contexts. "Let it go" at bedtime might be comfort-seeking. "Let it go" when a toy is taken might be protest or coping.
Your SLP can use context to understand:
- → What your child was communicating
- → Which situations trigger language
- → Where flexibility is emerging
You don't need to analyze it. Just describe what was happening.
Tips for quick capture
Use your phone

Don't overthink it
A rough note is better than no note. You can always edit later or add clinical structure.
Keep tags simple
A few consistent tags work better than dozens of specific ones. Think: location, emotion, source.
What to capture (and what to skip)
✓ Worth capturing
- New phrases or combinations you haven't heard before
- Familiar scripts used in new contexts
- Phrases that show clear communicative intent
- Moments that feel like progress (even if you're not sure why)
- Requests, protests, or emotional expressions
○ Okay to skip
- The same script repeated exactly as usual
- Self-stimulatory vocalizations without clear meaning
- Every single utterance (you'd never stop typing)
- Moments you're not sure about (unless you want SLP input)
When in doubt, capture it. Your SLP can help interpret. A "probably not important" entry might reveal a pattern when combined with others.
